INSTRUCTIONS FOR OBTAINING AN APPLICATION FOR MINOR WORK PERMIT
The process for the pre-application is as follows:
• The attached forms should be completed in its entirety by each party
and signed. The employer’s Tax ID Number is mandatory.
Unsigned forms will not be accepted by the school.
• The minor should return the completed form to the designated
school personnel who will then issue the minor work permit based in part on
the information contained on the pre-application.
